This form includes your organization's information, your authorized persons for ordering and/or receiving our consumer reports and drug test results, and the required notice to you as an "end-user" of consumer reports that allows us to provide you with FCRA-compliant background checks. This account also sets you up for our Live Scan (Level 2) fingerprinting services.
The Drug Screen form is for customers who only want our drug testing services and don't need background checks. This application also sets you up for our Live Scan (Level 2) fingerprinting services.
This simple, abbreviated form is for entities that only want to establish an account for fingerprinting services and not for background screening or drug testing.
